The Aged & Disability Services Unit (A&DS Unit) provide high quality, flexible and responsive services to more than 5000 peninsula residence who are older or living with a disability, supporting them to live safely, actively and independently in their own home and community. Twelve Senior Citizen centres, an industrial kitchen and community transport options are also managed by the Unit.

About the role:

The Home-Based Services Team is seeking qualified Community Care Workers for casual positions to provide individual assistance and care to older people, people with a disability and their carers’ assisting them to live independently in their own homes and in their local community.

Specifically, the position is required to

  • Provide a high level of client focused service delivery.
  • Deliver a range of responsive care and support services as per assessed client’s needs and under the direction of the Team Leader Home Based Services to ensure their wellbeing and to help them live independently in their own home.
  • Follow all A&DS policies and procedures relating to the Community Care Worker role, including Community Care Worker Handbook and Risk Control Measures.
  • Monitor clients’ health and wellbeing and identify client issues through observation and listening.
  • Report identified issues to the Team Leader Home Based Services for follow up and action within appropriate timelines as per phone call / client feedback forms.
  • Be responsive to diverse social, cultural and physical needs of clients and work within professional boundaries.

It is imperative that potential candidates have

  • An understanding of the issues confronting older people and people with a disability.
  • Knowledge of hygiene procedures in the maintenance of client home environments.
  • Knowledge and understanding of Occupational Health and Safety practices.
  • The functional capacity to lift and carry items as the duties are physically demanding.

The following are required upon commencement:

  • Certificate III in Home and Community Care, Aged Care or equivalent.
  • Previous relevant work experience in the Aged & Disability Services Industry.
  • Employee Working with Children Check. 
  • Australian Criminal History Check Clearance.
  • Functional Capacity Assessment Clearance (Facilitated by the Shire).
  • Victorian Drivers Licence and reliable vehicle with insurance.
